Assessment & Care Plan

The Assessment

Everyone starts their Brightly journey with a whole of person assessment. The assessment is centred around key domains of health covering physical, home & environment, cognitive, emotional, social, legal and financial wellbeing.

  • Fill out as much of the online assessment as you can. Alternatively, you can skip straight to Step 2.

  • One of our friendly healthcare assistants will get in touch to confirm details and book you in for a consultation with one of our experienced nurses.

  • Next you’ll have a consultation with a Brightly nurse. We will go over your assessment, do some physical tests and go over a few more in detail questions.

The Care Plan

Following your nurse consultation, your nurse and the Brightly Care Team, will develop a personalised Care Plan for you. The Care Plan includes a summary of your assessment and recommendations for positive action, including details specific to you.

Brightly
Membership

The Brightly Membership offers full support to assist you in getting the most out of your Care Plan.

Your Membership includes:

  • Access to our online portal

  • Access core wellbeing modules on diet, exercise and sleep

  • Ability to track your progress from your first assessment and keep tabs on your “to do” list

  • Concierge support

Members also get preferred booking and discounted appointments with our Brightly Specialist Network.

Brightly
Care Team & Specialist Network

Care Team

Brightly’s in-house care team includes: geriatrician, GP, physiotherapist, occupational therapist, social worker, dietician, healthcare assistant, nurse and concierge.

Our Care Team is involved in creating personalised Care Plans and are available to Brightly Members for support.

Specialist Network

When more specialist or condition-specific input is required, we connect members to best-in-class experts through the Brightly Network. These services include: optometry, dental, audiology, podiatry, legal and financial.
