Brightly HealthPlan™ is a flexible membership model designed to support people at different stages of health, independence, and complexity.
Brightly members range from people who are pre‑frail, with no obvious ageing‑related symptoms, through to those with high frailty and an increased risk of hospital or rest‑home admission. Every HealthPlan provides the base level of clinical care, escalation pathways, and specialist access that everyone needs to remain safe, supported, and independent.
As health needs change, additional services can be introduced. These targeted services are charged separately and may include medication reviews, geriatrician home visits, home independence audits, driver licensing renewal coaching, downsizing support, and other specialised interventions when required.

Brightly Core is designed for people who are comfortable managing their health independently, with structured clinical guidance, regular review, and clear escalation pathways.

- Geriatrician‑approved prevention priorities and key focus areas
- Personalised Care Plan with measurable goals and agreed biomarkers
- Identification of emerging health risks and key life milestones
- Evidence‑based strategies to support independence and wellbeing
- Six‑monthly nurse‑led reviews to update Health Score and Care Plan
- Ongoing risk monitoring and targeted intervention planning
- Cognitive training to support brain health
- Access to Brightly’s multidisciplinary team
- GP liaison when required
- Priority access to, and coordination of, specialist health services when needed
