MindThrive is a three-month clinical programme that investigates and targets every one of these factors — with eight clinicians, a fully coordinated care pathway, and a clear outcome at the end.

Most memory assessments look at cognition in isolation. MindThrive looks at a broad range of factors that could be driving or accelerating cognitive change, and systematically investigates and targets every reversible factor.

 HOW THE PROGRAMME WORKS 

Three months. No stone unturned.

MindThrive follows a structured three-step pathway across three months, with a clear clinical decision at the end about your next steps.

Initial consultation: your full intervention plan is built and activated

Your Brightly GP reviews your complete Baseline results and identifies every reversible risk factor relevant to you. All parallel interventions are activated in this single consultation — cardiovascular, metabolic, and mood management plans are initiated, and referrals to hearing and sleep specialists are coordinated immediately. This is where the full three months of work is set in motion.

 WEEK 1–2 

Parallel interventions: eight clinicians working simultaneously across three months

Your cardiovascular, metabolic, and mood management plans run throughout the programme. Your Integrative Doctor prescribes a brain-targeted supplement protocol based on your blood data. Your dietician ensures that your nutrition is in check. Your Health Coach helps keep you accountable to your brain health plan.

If you opt in for a comprehensive Medication Optimisation ($1,299), your Pharmacist conducts a full medication review. External audiology and sleep specialists are seen, with results returned to your GP for integration. All eight members of your Brightly Neuro team work in parallel — not sequentially.

 MONTHS 1–3 

Three-month review: your progress is reviewed and next steps are agreed

Your Brightly Brain Health Nurse conducts a structured review of your memory concerns and clinical progress at the end of three months. Your Geriatrician reviews all findings and recommends your pathway — exit MindThrive with a clear lifestyle plan and continued support if reversible factors have been successfully addressed, or progress to a formal diagnosis pathway if concerns remain.

ACCOUNTABILITY & IMPLEMENTATION

Your health coach works with you on whatever matters most for your own situation — whether that's making lasting changes to your diet, improving your sleep, building an exercise routine you'll actually stick to, managing stress, or anything else that came up in your first session together.

HEARING

Assesses hearing function via coordinated external referral — untreated hearing loss is one of the strongest modifiable dementia risk factors. Results are returned to your Brightly GP for integration.

(If needed) Conducts a full review of all your current medications — identifying interactions, contraindications, or any drugs that may be contributing to cognitive symptoms.

(If needed) Assesses for obstructive sleep apnoea via coordinated external referral — untreated OSA is significantly associated with cognitive decline. Results are returned to your Brightly GP for integration.

At your three-month gate review, your Brightly Neuro Nurse and GP recommend your pathway. You will not be left with uncertainty about what your results mean or what to do next.

Reversible causes found and addressed — move into our Brain Health Membership with confidence

If your review shows that the reversible factors driving your cognitive concerns have been successfully identified and addressed — and that no further concerns remain — you exit MindThrive with a clear lifestyle plan. Your ongoing care continues through Brightly, with your cognitive health monitored at every reassessment. 

PATHWAY B – IF CONCERNS REMAIN

Referred into a formal diagnosis pathway — with your team beside you

If concerns remain at three months despite addressing all reversible factors, your GP will refer you into a formal diagnosis pathway — geriatrician-led, fully coordinated, and designed to give you and your family a clear, complete clinical picture of what is happening and what comes next. Your Brightly team remains with you throughout.

MindThrive is available to Brightly members who have completed their Baseline assessment. Your Baseline results are the clinical foundation on which your intervention plan is built.

WHAT'S INCLUDED

A consultation with your Brightly GP, who reviews your results and builds your personal 3-month plan

Ongoing coordination from your Brightly nurse to help set up any referrals and keep things moving

Four sessions with your health coach, to help you make real changes to your nutrition, sleep, movement, stress and alcohol intake

Extra nurse support along the way, if your health coach thinks you'd benefit from clinical advice

A repeat memory assessment and blood tests at the end of the programme, to see what's changed

A geriatrician's review of all your results

A final consultation with your Brightly nurse to walk you through your results and your next step — either ongoing lifestyle support, or a referral for a formal diagnosis if any concerns remain
