Follow-Up Session

A focused check-in to see how your plan is working, adjust what needs adjusting, and troubleshoot anything that's not landing. Same coordinated team, same continuity.

- Direct communication with your geriatrician and health coach, so changes to your nutrition plan stay consistent with the rest of your care.
