Consent for recording and use of testimonial

Purpose

After your assessment, we may invite you to share your feedback about your experience with Brightly.

If you agree, we’d like to record it and use it to help explain Brightly to others.

This is completely optional. Saying no will not affect your access to Brightly services or your care plan.

What you are agreeing to

By signing this form, you agree that Brightly may:

record your feedback (audio/video/photos) and may make written notes of what you say; and
use your recorded feedback (including your image and voice) in Brightly materials such as our website, social media and other promotional or educational materials.

We may edit your testimonial for length or clarity, but we won't change the overall meaning.

Privacy and health information

I understand that my testimonial may include information about my health or wellbeing and that this is personal/health information. I understand that my information will be collected, stored, and used in line with Brightly’s Privacy Policy, and that New Zealand privacy and health information laws apply.

Your choices and rights

You may withdraw your consent at any time by contacting us at privacy@agebrightly.co.nz or by telling a member of the Brightly team. If you withdraw consent, we will stop using your testimonial in new materials and where reasonably practical, will remove it from our website, app, and social media channels.
