Brightly Baseline+™ Terms

These terms apply when you book and receive a Brightly Baseline+™ assessment (Baseline+).

Key points

  • Baseline+ is the starting point for Brightly. It is the whole-of-person assessment we use to understand your health, risks, goals and priorities before your Brightly HealthPlan™ membership begins.

  • Following Baseline+, Brightly will build your personalised HealthPlan™ and, unless otherwise stated at booking or agreed with you, your HealthPlan™ membership will begin in accordance with the HealthPlan™ Terms.

  • What Baseline+ includes is described on our website. This may include review of physical health, cognition, medications, mobility, sleep, nutrition, future planning, biomarkers, data points and other matters relevant to your assessment.

  • You can reschedule; late changes/no-shows may incur a reasonable fee.

1. Booking and consent

If you book for another person, you confirm that you have their permission to make the booking and provide their details. We’ll still need to involve the person being assessed in the onboarding (for example, to confirm their details and check they’re comfortable for us to proceed). If they can’t give consent themselves, we can only continue if someone who is legally allowed to make health decisions for them gives consent on their behalf (for example, an attorney under an Enduring Power of Attorney for personal care and welfare).

2. Privacy and information sharing

We collect personal and health information to provide Baseline+ (including coordinating any tests and communicating results and recommendations). With your authority, we may request information from your GP/other providers and share relevant information with third parties involved in your assessment (e.g., a laboratory). 3. Fees, changes, cancellations and refunds

Promotions available from time to time, such as discounts for spouse bookings, are shown on our website. Fees are as shown at checkout. Payment is due as stated at checkout. If you cancel or reschedule with less than 48 hours’ notice, or do not attend without notice, we may charge a reasonable fee reflecting the time reserved for you. If we need to reschedule, we’ll offer the next available time.

If we can’t deliver Baseline+ for reasons within our control, you can choose a reschedule or refund. Once Baseline+ has been delivered (including results/recommendations), fees are not refundable except where required by law.

We may help you understand whether parts of your care may be eligible for private insurance cover or public funding. This support is general coordination support only. We do not guarantee that any service will be covered, reimbursed or funded, and decisions about cover or funding are made by the relevant insurer, public agency or provider. Unless we state otherwise, the Baseline+ programme is not covered by private health insurance. 

4. Liability and legal rights

Nothing in these terms limits any rights you have under New Zealand law (including the Consumer Guarantees Act 1993, the Code of Health and Disability Services Consumers’ Rights, the Privacy Act 2020 and the Health Information Privacy Code 2020). To the extent permitted by law, Brightly is not liable for indirect or consequential loss arising from your booking or participation in Baseline+. 

5. Membership (optional)

After Baseline+ you may choose to join a Brightly membership for ongoing support. If you do, separate membership terms and pricing will apply and will be provided before purchase.
